What documentation must I submit with my Draw Request?

Homeowners must submit the following documentation in the online portal as requested in the "Draw Request" steps: 

Matching Grant Draw Request Documentation: 

• Original Contractor Invoice - a detailed invoice or quote that describes the mitigation project(s) that was completed including any relevant change orders or extra costs. 
• Proof of Payment in Full - examples of proof of payment could be: 
  • An invoice marked as paid in full and dated. 
  • Cancelled checks (image of front and back)
  • Receipts
  • Bank statements
  • A financing agreement including the name of the contractor. 
• Proof of Insurance Premium Discounts, if any - documentation that the final inspection report has been submitted to the insurance company. Some examples are:  
  • A new declarations page that reflects change in premium amount due to discounts received from completing mitigation project(s). The document submitted must show an effective date or amended date after the final inspection report. 
  • A letter or an email from insurance provider stating the change in premium amount due to discounts received from completing mitigation project(s) or stating that discounts will not be applied.
  • A quote from insurance provider showing new premium amount.

Low-Income Draw Request Documentation:

  • A new declarations page that reflects change in premium amount due to discounts received from completing mitigation project(s). The document submitted must show an effective date or amended date after the final inspection report. 
  • A letter or an email from insurance provider stating the change in premium amount due to discounts received from completing mitigation project(s) or stating that discounts will not be applied.
  • A quote from insurance provider showing new premium amount
  • A statement that they do not have insurance if they cannot provide a current declarations page.
