What are the steps to submit the Prioritization Questionnaire?

Before Prioritization Questionnaire: 

  • A Hurricane Mitigation Initial Inspection must have been completed and the Initial Inspection Report uploaded into the Applicant Portal. 

Submitting the Prioritization Questionnaire: 

Homeowners must enter the Applicant Portal and complete the following steps to ensure proper prioritization for future Grant Application opportunities: 


1. "Questionnaire - Program Overview" – contains an overview of the My Safe FL Home Program and provides link to the MSFH Homeowners Guide.  

  • Carefully review the provided information on the Program introduction and requirements. 
  • Click the link provided to review the MSFH Homeowners Guide. 
  • Confirm reading the Homeowner's Guide and information provided on the "Program Overview" step by clicking on the checkbox.
  • Select "Complete & Continue" on the step to move forward. Individuals will not be able to edit this information once the step has been completed.

2. "Questionnaire - Homeowner Information" - contains important demographic information requested to be filled in by the Applicant. 

  • Enter the demographic information such as Applicant's name, property address, insurance provider, property information, county of residence, and alternate contact information. 
  • Double-check the information provided to confirm it is correct before selecting "Complete & Continue" on the step to move forward. Individuals will not be able to edit this information once the step has been completed. 
  • Select "Complete & Continue" on the step to move forward. Individuals will not be able to edit this information once the step has been completed.

3. "Questionnaire - Household Members" - contains household information to be filled in by the Applicant. 

  • Enter the first name, middle name (optional), last name, and birthdate of the "Primary Household Member." PLEASE NOTE: The driver's license number listed on the upcoming "Confirm Prioritization Questionnaire" step MUST be for the "Primary Household Member" as identified in the "Household Members" step. 
  • Enter the first name, middle name (optional), last name, and birthdate of all additional "Household Members." A Household Member is someone who claims the property as their permanent residence. PLEASE NOTE: People who are under 18 who live at home must be included as a "Household Member." Household Members do not necessarily need to contribute income to be included as a "Household Member".
  • Double-check the information provided to confirm it is correct before selecting "Complete & Continue" on the step to move forward. Individuals will not be able to edit this information once the step has been completed.

4. "Questionnaire - Income" - contains annual income information to be filled in by the Applicant.

  • Click the link to review instructions on determining the Adjusted Gross Income.
  • Select which "Household Income Certification Method" to utilize. 
  • Enter the annual household income using one of the Household Income Certification methods: either 1) enter the entire household's Adjusted Gross Income as a lump sum, or 2) enter the Adjusted Gross Income per household member and let the system calculate the total.
  • Double-check the information provided to confirm it is correct before selecting "Complete & Continue" on the step to move forward. Individuals will not be able to edit this information once the step has been completed.

5. "Questionnaire - Confirm Prioritization Questionnaire" contains declarations that all information provided in Prioritization Questionnaire steps is true and accurate, and requests Applicants enter the driver's license number or Florida State Identification Card Number for the Primary Household Member. 

  • Read the statement regarding the definitions of Low-Income or Moderate Income and select the box to declare that the income and family size information provided is true and accurate. 
  • Select "Yes" or "No" if any owner of the property is aged sixty years or older. 
  • Enter the driver's license number or Florida State Identification Card Number for the Primary Household Member. PLEASE NOTE: The driver's license number listed on the "Confirm Prioritization Questionnaire" step MUST be for the "Primary Household Member" as identified in the "Household Members" step of the Prioritization Questionnaire in the Applicant Portal. (If one or more Household Member(s) is aged sixty years or older, this individual MUST be listed as the Primary Household Member.)
  • Read the statement regarding prioritization and affirm that the information provided is true and correct. 
  • Double-check the information provided to confirm it is correct before selecting "Complete" to submit the Prioritization Questionnaire. Individuals will not be able to edit this information once the step has been completed. 

After Submitting the Prioritization Questionnaire: 

After Homeowners have updated information in the Applicant Portal, they are encouraged to regularly check the My Safe FL Home website at https://mysafeflhome.com/ for upcoming opportunities to apply for a Hurricane Mitigation Grant. Homeowners should also regularly monitor their case in the Applicant Portal for any case status updates. PLEASE NOTE: Receiving a Hurricane Mitigation Initial Inspection and entering demographic information in the Prioritization Questionnaire is NOT considered the Grant Application. The availability of the Grant Application itself will be announced at a later date.
